Job Summary and Responsibilities
As our Community Health Advocate, you will help individuals and communities adopt healthy behaviors and connect with vital resources so they can improve their overall health and well-being.
Every day you will conduct outreach, provide information, and offer support to promote health initiatives within the community. You will be expected to advise clients on a range of health topics, from disease prevention to securing stable housing, and act as a crucial link between community members and essential health services.
To be successful in this role, you must possess strong communication and interpersonal skills, a deep understanding of community needs, and the ability to advocate for individuals and groups seeking support.
- Advises clients or community groups on issues related to diabetes and obesity, improving general health, diet and exercise, prevention, sanitation or hygiene, self care, social issues, education, childcare or problem solving.
- Acts as a resource to all clients, providers and the community.
- Attends community meetings and health fairs to understand community issues and build relationships with community members.
- Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.
- Refers community members to needed health services.
- Advises clients or community groups to ensure parental understanding of the importance of childhood immunizations and how to access immunization services.
Minimum
- High School Graduate or High School GED
- Experience working in the community
- Driver's License:NV (AUTO:Nevada)
Preferred
- Bachelors, other and teaching experience
- Community Housing support experience
