Responsibilities

Is expected to perform duties in alignment with the mission and policies of the organization. Performs duties in support of and in compliance with the performance improvement plan Joint Commission and other licensing accrediting and regulatory agencies. Through screening mechanisms identifies patients who are at high risk for need of social services intervention. Provides assessments to identify bio-psychosocial emotional substance use maturational cultural adjustment financial and/or legal problems then intervenes by implementing suitable interventions. Facilitates linkage/assistance that enables patients/families to access medical care to achieve optimum levels of health and fulfill goals for treatment. Functions may also include discharge planning.One to three years experience in a medical facility with acute or outpatient psychiatric experience preferred. One to three years experience in an acute care setting as a Medical Social Worker. Graduation from an accredited School of Social Work with an MSW degree. Additional Requirements: functions independently prioritizes and organizes workload effectively capable of critical thinking and executing decisions makes recommendations to an interdisciplinary team builds trust and respect among peers and other departments participation as a team player has the knowledge and skill to work well with all inpatient populations. Licensed to function independently as a clinical social worker; LCSW.


Qualifications
  • At least two-years of clinical and/or medical social work experience required in an medical facility.
  • Prefer one year of clinical and/or medical social work experience in an acute psychiatric facility.
  • Masters Degree from a School of Social work accredited by the Council of Social Work Education.
  • Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse neglect or violence; community or financial resources for underserved or vulnerable populations.
  • SSDI SDI and other governmental/county requirements/programs.
  • Thorough comprehension of OBRA and Title 22 is required.
  • Excellent customer service and presentation skills are a must Strong interpersonal and written communication skills are essential Demonstrated ability to apply analytical and problem solving skills Demonstrated ability to manage multiple tasks or projects effectively Ability to work independently as needed with a high degree of detail orientation.
  • Ability to work efficiently in a fast-paced environment with changing priorities.
  • Licensed Clinical Social Worker (LCSW) Required
