Responsibilities

Parkview Health at Home Now Hiring LPN Home Health

Mon -Fri (8:00 AM - 5:00 PM) 

  • No on-call requirement with flexible schedules.  
  • Provides care and educates patients and their caregivers
  • Coordinates services with other disciplines and community resources as required
  • Evaluates and updates care plans and assessments in response to patient and environmental changes
  • Attends regular case conferences, in-services, and special case orientations as required, and participates in on-call rotation

Qualifications

  • Active LPN License
  • One year of clinical experience as a LPN required
  • Valid Driver’s License
  • Proficient in technology, including smartphones, tablets, virtual reality, interactive software, and artificial intelligence
  • In Washington State, all providers must meet the National Provider Identifier (NPI) requirement and provide proof before onboarding.
