At CHI Health at Home, we are dedicated to making a difference in peoples lives every day. We provide supplemental home health staff in Fort Wayne, Indiana.  We are offering an LPN  day shift assignment providing home health services throughout Fort Wayne.  


Responsibilities

Perform patient comprehensive assessments, collaborating seamlessly with our multi-disciplinary team of experts, including RN, PT, OT, ST, MSW, and paraprofessionals. Together, we will work passionately to achieve the heartfelt outcomes and goals of our patients, leading them on a journey of healing and hope.

Lead with Heart - Nurturing Care Coordination

Work your caseload, nurturing the growth and development of  Home Health Aides, as we cultivate a harmonious team of caregivers. 

With us, your diligent documentation paints a vivid picture of our patients' journeys towards wellness. With the ease of a tablet, you capture the essence of OASIS assessments, SOC/Admissions, recertification, physician orders, and more - weaving together a tapestry of care that surpasses expectations.


Qualifications
  • Active LPN License
  • One (1+) Year of Clinical Experience
  • CPR Certification
  • Valid Driver's License
