RN Coordinator Wound Care

Position Summary

The Wound Care Coordinator is responsible for the development coordination and monitoring of the hospitals Wound Care Program. The Wound Care Coordinator establishes standards of care and clinical competencies to meet patient needs and training to ensure clinical expertise. The Wound Care Coordinator works collaboratively with the medical staff and provides education to patients caregivers and staff on wound ostomy and continence care. This position requires the full understanding and active participation in fulfilling the Mission of Mercy Hospital of Folsom. The employee shall support Mercy Hospital of Folsom’s strategic plan and the goals and direction the quality improvement / process improvement activities.

Position Scope

• Develops and implements the hospitals Wound Care Program to include patient care protocols documentation tools wound care formulary and WOCN referral criteria.
• Establishes standards of care competencies policies and procedures in quality cost-efficient and effective wound care for all clinical staff.
• Makes recommendations to the medical staff for changes to wound care orders and provides evidence-based research support as needed
• Provides instruction and guidance both formally and informally to staff and assists in identifying staff learning needs related to wound ostomy and continence as appropriate.
• Assures that new equipment and procedures are inserviced to all staff members.
• Performs weekly skin/wound rounds on selected units/patients
• Provides wound or ostomy consultation at the request of any multidisciplinary team member.
• Monitors/approves rental equipment in relation to pressure ulcer prevention and treatment and wound management.
• Collaborates with Case Management for patient referral as needed at discharge.

Minimum Required

• Current CA RN licensure and AHA BLS
• Bachelor’s Degree in Nursing (BSN)
• Three (3) years experience as a Registered Nurse in an acute setting in the last 5 years.
• RNs with subacute experience may be considered based on the complexityof wound care performed within the subacute setting
• Within 24 months of hire the following must be obtained:
  • Wound Ostomy and Continence Certification (WOCN) or Ostomy Management Specialist Certification (OMS) with Certified Wound Care Nurse Certification (WCC)
