The posted compensation range of $30.70 - $44.55 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Responsible for coordinating and implementing post-discharge plans in coordination with the Case Managers through the use of Extended Care Information Network (ECIN). They are also responsible for assisting with advocacy and referrals to other community resources. We prefer candidates with experience working with substance use disorder, harm reduction, palliative and end-of-life care, and knowledge of local resources
Job Requirements
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
- Master Degree Social Work
- Knowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills, knowledge of third party payment systems, Medicare/Medicaid programs.
- Maintains current knowledge base of community services through continuing education.
- Ability to multi-task, set priorities and maintain organization.
- Computer skills.
- LCSW Required
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly.
This is a PRN Social Worker role that works variable shifts. Applicants need to be able to work weekdays, weekends, and holidays in Durango, Colorado. We are looking for an LCSW who has experience with trauma-informed behavioral health assessment and brief intervention, is easily able to work on a multidisciplinary team, has crisis intervention, de-escalation, and problem-solving skills, is able to manage time and multitask, and is willing to crosstrain in oncology.
