Responsibilities
  1.   2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
  3. Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.
  4. Establishes a care plan for continued needs based on the client assessment.
  5. Reports changes in patient condition to appropriate personnel and the attending physician.
  6. Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers.
  7. Participates in coordination of services with other disciplines and community resources as needed.
  8. Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.
  9. When providing private duty or staffing in a facility, follows the sign-in procedure and the policies andprocedures for that particular institution.
  10. Presents patient assessment information to the supervising physician.
  11. Performs appropriate emergency measures as deemed necessary
  12. Assists with the development of educational programs in the palliative care program for diverse age groups (i.e., infancy, childhood, adolescence, adult and geriatric).
  13. Provides paraprofessional supervision as required by policy and regulations
  14. Completes and submits all required documentationin a timely manner.
  15. Regulates and adjusts medications as indicated.
  16. Demonstrates sensitivity to patient and/or family needs, customs or feelings.
  17. Participates in case conferences.
  18. Recognizes emergencies and has knowledge of emergency procedures.
  19. All other duties as assigned.

Qualifications

Graduate of an accredited school of nursing.

Licensed to practice in Colorado. 

One year of nursing experience required.

Overview

The Nurse Practitioner provides face-to-face assessment services to patients to assist the Medical Director and Team to determine each patient’s continuing eligibility for palliative care.  May provide service in various settings, including the patient’s home, a physician’s office, the inpatient palliative care facility, etc.
