The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes
effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their
caregivers.

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

• Bachelor Degree Nursing Required
• Three years of experience required, with one year in specialty.
• Strong psychosocial assessment and clinical skills.
• Registered Nurse State of Employment Required

• Basic Life Support American Heart Association Required

• American Nurse Credentialing Center Nursing Professional Cert Preferred
