The posted compensation range of $47.4107 - $70.52 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Job Summary and Responsibilities
This is a PRN position
As a Nurse Practitioner with CommonSpirit Health at Home, you’ll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient’s continuing eligibility for palliative care.
Responsibilities:
- Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.
- Establishes a plan of care for continued needs based on the client assessment.
- Reports changes in patient condition to appropriate personnel and the attending physician.
- Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers.
- Participates in coordination of services with other disciplines and community resources as needed.
- Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.
- Assists with the development of educational programs in the palliative care program for diverse age groups.
- All other duties as assigned.
Job Requirements
- Graduate of an accredited school of nursing.
- Currently licensed to practice in the state serviced, with Masters in nursing.
- At least 1 year of nursing experience
- Must obtain and maintain current CPR
- Strong critical thinking, clinical expertise, case management, and a passion for patient care.
- Valid driver’s license and auto insurance coverage according to the CHI Health at Home policy.
