  • Offer crisis support, counseling, and resource referrals for patients and families

  • Help plan safe discharges and transitions of care

  • Coordinate with healthcare teams to keep patients supported and informed

  • Connect patients to community resources for housing, food, transportation, and more

Job Requirements

Education:  Master's Degree fom a school of social work accredited by the Council of Social Work Education (MSW) REQUIRED
