Responsibilities

Join American-Mercy Home Care as a Medical Social Worker, where you will be at the forefront of coordinating care and fostering a supportive environment that enhances the social, emotional, functional, and physical health of our patients.

As a Medical Social Worker, you will play a crucial role in establishing care plans in line with the physician's treatment strategy. Your expertise will be the cornerstone of personalized care, ensuring that every patient's unique needs and circumstances are carefully considered.

In times of crisis, you will be the pillar of support for patients and their families, providing much-needed intervention and understanding. Your compassion will make a world of difference in their healing journey.

Our patients' lives are often influenced by a myriad of social, emotional, and economic factors. With your insight, you will help physicians and caregivers understand and address these elements that impact the patient's medical progress.

Qualifications
  • Master's degree from accredited school approved by Council on Social Work Education. 
  • Must be certified through the Board of Social Work in the state of service.
  • One year of social work in a health care setting is preferred.
  • Must obtain and maintain CPR Certification.
