Job Summary and Responsibilities
As a Patient Flow Nurse, you will optimize patient movement throughout the hospital, ensuring efficient transitions between departments, units, and levels of care. Your role minimizes wait times and maximizes bed utilization.
Every day you will assess bed availability, patient acuity, and discharge readiness, proactively managing admissions, transfers, and discharges. You'll collaborate with multidisciplinary teams, resolving logistical challenges and prioritizing patient movement based on clinical urgency and resources.
To be successful in this role, you need strong critical thinking, a comprehensive understanding of hospital operations, and exceptional communication. Your calm, swift decision-making and mediation skills in dynamic situations are essential for seamless patient progression.
- Accurately assesses patient information (condition, admitting diagnosis, etc.) to determine appropriate placement within the hospital, ensuring timely and efficient bed assignment. This includes utilizing established criteria for admission appropriateness.
- Effectively manages inbound and outbound communication with physicians, other healthcare providers, and internal departments via phone, electronic health record (EHR - Epic), and other communication systems (Teletracking). This includes timely responses to inquiries and efficient dissemination of critical information.
- Communicates critical test results to appropriate healthcare providers and facilitates necessary follow-up actions, including scheduling of emergent surgeries (after-hours) and making follow-up phone calls as needed.
- Collaborates with physicians to facilitate patient hand-offs, providing clear and concise information. Provides support for curbside consults and assists with the timely resolution of clinical issues.
- Accurately documents patient information and communication in the EHR (Epic) and other designated systems (Teletracking). Maintains up-to-date knowledge of hospital policies and procedures related to patient flow.
- Accurately receives and documents verbal orders from physicians, ensuring clarity and completeness.
Required
- Graduate from an accredited Nursing program, upon hire
- Registered Nurse: TN, upon hire
Preferred
- Bachelors Other Nursing
- Experience in bed management
- EMR systems
