Job Summary and Responsibilities

As our Licensed Medical Social Worker at Chandler Regional Medical Center, you will perform critical social work assessments and interventions for inpatient, outpatient, and Emergency Department patients. You'll lead patient-centered care by developing and implementing treatment plans, facilitating post-acute placements, and coordinating comprehensive resources for complex discharges and vulnerable populations. This role is central to crisis intervention, patient advocacy, and ensuring developmentally appropriate care across all served populations, from mentally ill to chronically ill.

Every day you will manage psychosocial care, including crisis intervention, counseling, and referrals for diverse issues like abuse, family challenges, and grief. You'll serve as a patient advocate, educating on self-care, bioethics, and insurance, while actively coordinating community resources to address social determinants of health such as homelessness. You'll also contribute to performance improvement teams, ensuring adherence to federal, state, and regulatory guidelines to enhance financial outcomes and reduce readmissions.

Your ability to independently manage multiple priorities, work efficiently in a fast-paced environment with changing demands, and collaborate effectively within an interdisciplinary team is crucial.   • Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
  • Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families 
  • Serve on hospital committees and develop and present in-services to Hospital staff
  • Participates in performance improvement teams and programs as necessary
  • Demonstrates behavior that aligns with the Mission and Core Values of the Organization

Job Requirements

Required

  • 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
  • LMSW license from the AZ Board of Behavioral Health Examiners 
  • Master's Degree from a School of Social work accredited by the Council of Social Work Education
  • Ability to work independently as needed with a high degree of detail orientation
  • Ability to work efficiently in a fast-paced environment with changing priorities 
  • Ability to work collaboratively with an interdisciplinary care team

Preferred

  • Minimum 3-Year Post-MSW healthcare experience
