Job Summary and Responsibilities

Job Summary:

This position plans, implements, and coordinates Community Wellness Healthier Living workshops and programs that include education, coaching, mentoring and support of chronic disease management. In
addition, this position assists with reporting, development, data collection, volunteer management and evaluation of Healthier Living workshops.

Responsibilities:

Facilitates Healthier Living Workshops
Attends workshop Meet and Greets and other promotional activities
Monitors program phone and Email, and responds to questions and concerns
Processes workshop registrations
Contacts registered participants with a workshop reminder
Maintains inventory of workshop equipment and supplies
Oversees the compilation of workshop material and equipment for program facilitators
Recruits volunteers
Markets the programs to stakeholders, including planning and attendance at health fairs, special events, and representing the department and organization for Healthier Living programs
Collaborates with internal and external stakeholders for strategic growth of program

Program Fidelity:

Obtains and maintains certification in Healthier Living workshops as requested
Monitors and maintains program fidelity standards
Orients new facilitators and ensures adequate socialization to the role and to the department

Administrative Duties:

Collects data as assigned to evaluate program effectiveness
Prepares class schedules, assuring partner locations and facilitators are secured
Assures equipment is functional and supplies/equipment are stocked for workshops and health fairs
Ensures attendance records and surveys are collected in a timely manner
Assumes responsibility for required training modules, certifications, professional development and continuing education
Assists director with revision in program workshop format, curriculum
Assists director with strategic planning
Assists director with input into grant applications


Job Requirements

Minimum:

Experience:

Certification in one type of Healthier Living Workshop: Chronic Disease, ChronicPain, Diabetes Self Management, Diabetes Empowerment Education Program (DEEP) or Matter of Balance.
May obtain certification after start date, but must do so within eight weeks.

Preferred: Experience in: public speaking, adult learning concepts, and healthcare.

Education:

High School Diploma/GED

Additional Skills:

Must have effective verbal and written communication skills. Able to use email, and other office-related software programs, including virtual platforms.

Located in the Phoenix, Ariz., suburb of Chandler, the Chandler Regional Medical Center is a 429-bed, not-for-profit hospital with more than 2,600 team members and almost 1,000 doctors on staff representing all major specialties. Chandler Regional joined the Dignity Health system in 1999, but has been part of the community for more than 50 years. The hospital includes a Level I Trauma Center, a Heart and Vascular Center, a Wound Healing Center, a Family Birth Center and many additional services. Dignity Health Chandler Regional, Mercy Gilbert, and Arizona General Hospital Mesa awarded $422,000 in grants to support five community projects led by 15 local organizations. Since 1991, Dignity Health's Community Health Improvement Grants program has provided $91 million to nearly 3,800 health projects, focusing on vulnerable and underserved populations.
