Job Summary and Responsibilities
The Pharmacy Department at Chandler Regional Medical Center operates 24/7, providing innovative and automated pharmaceutical services across all hospital departments. Our team consists of decentralized and centralized pharmacists, along with specialists in infectious disease and cardiology. Pharmacy technicians support pharmacists by utilizing the Omnicell automated dispensing machine, Cerner patient care system, and bedside medication verification.
We are seeking an exceptional pharmacist for a hybrid role primarily focused in our Critical Care areas (ICU/ED). This pharmacist will deliver comprehensive clinical pharmacy services, including pharmacokinetic and therapeutic consultations. Responsibilities include collaborating with the interdisciplinary team to ensure optimal medication management, reviewing orders, monitoring therapies, and managing drug therapy during emergencies. The pharmacist will perform accurate and timely computer order entry and medication reviews for therapeutic appropriateness, duplications, interactions, effectiveness, and safety. Additionally, this role provides an opportunity to mentor PharmD candidates and PGY1 residents, fostering their professional development within the pharmacy team.
- Review prescriptions in queue for safety and accuracy. Check patient’s medical history to ensure any medications prescribed will be safe and effective. Make recommendations if you see an error or know another medication would be better.
- Consults with and advises medical, nursing, and other healthcare providers regarding the prescribing, dispensing, or administration of pharmaceuticals
- Routinely monitors the patient’s total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities
- Conducts assigned target drug monitoring programs and concurrent drug use evaluation activities as required
- Performs and documents Pharmacist Intervention activities such as recommendations on drug product selection, dosage adjustments, and appropriate drug therapy
- Provide CommonSpirit’s level of exceptional service to all patients or nurses filling orders.
- Participates actively in cardiopulmonary emergencies.
Qualified candidates should have a PharmD degree, with residency training preferred, and strong clinical experience in critical care settings. Minimum:
Current Arizona State Pharmacist License in good standing
Good communication skills. Ability to work under stress.
Previous experience with Pharmacy computer systems.
