Job Summary and Responsibilities

The Supervisor is responsible for the oversight of day to day departmental operations of assigned department(s) including, but not limited to: clinical resource management, transitions of care across the continuum, patient advocacy, best practice in medical and social  necessity determination. This position will supervise assigned departments in an efficient manner to ensure timely and compliant patient services are provided resulting in quality patient care. The Supervisor may assign work tasks, realign tasks within the department and take action to resolve minor disputes and productivity issues. This position may be assigned other duties within scope of practice, including, but not limited to : scheduling, conducting audits, performance improvement activities, assistance in staff evaluations or assuming a case load based on departmental needs. The position's focus will be on optimizing staff productivity, collaboration and compliance . The Supervisor provides support and coaching to staff members as indicated. This position will report to and collaborate with the Manager or Director regarding identified issues outside the Supervisors scope of authority.The Supervisor guides department activities according to the organizational objectives, standards of practice and Federal and State Regulations and any accrediting body active within the team.

Job Requirements

Required Education and Experience:

  • BS degree or experience equivalent required
  • Minimum of 3 years in a care coordination, case management, quality or other outpatient clinical / operations experience

Required Minimum Knowledge, Skills, Abilities and Training:

  • Ability to manage and work closely with interdisciplinary partners in the management of identified patient populations. Demonstrated ability to oversee a mix of clinical, operational, and business activities related to the team(s).
  • Will need to be able to implement specific program goals including high priority project redesign efforts required to improve performance.
  • Ability to work in partnership with all applicable internal and external customers who are focused on services to our patients in order to ensure that patients' care along the continuum is seamless.
  • Ability to assess, and report patient status on a periodic basis to all team stakeholders.
  • Excellent computer skills and ability to learn new systems.
  • Strong organizational (time management) and  interpersonal skills.
  • Ability to communicate effectively using written and verbal skills. Proficient in email communications and internet usage along with basic use of office support platforms including documentation, spreadsheets and presentation software programs.
  • Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost). 
  • Knowledge of the content, intent and application of federal and state standards and regulatory requirements  as well as accreditation requirements pertaining to matters within designated scope of responsibility.
  • Ability to work autonomously within a matrix environment without direct supervision or support.
  • Ability to handle multiple priorities with strong attention to detail. 
  • Demonstrated ability to set goals either for themselves or others.
