Dignity Health Mercy San Juan Medical Center is a 370-bed not-for-profit Level 2 Trauma Center located in Carmichael California serving the areas of north Sacramento County and south Placer County for more than 50 years. Responsibilities
We are seeking a dedicated and experienced Home Care Supervisor to lead our large progressive Home Health Agency which serves a diverse client base throughout the Sacramento Service area. As a Supervisor, you will lead strong multidisciplinary clinical teams and play a vital role in ensuring the delivery of high-quality patient care.
Responsibilities:
Supervise and motivate an interdisciplinary team of home care professionals.
Act as a clinical resource, screening referrals and determining patient acuity levels.
Monitor staff performance, conduct evaluations, and implement corrective actions as needed.
Ensure compliance with all state, federal, and accreditation requirements.
Collaborate with physicians, interdisciplinary teams, and community resources to optimize patient care.
Assist in the development and implementation of patient/family educational materials.
Stay abreast of healthcare trends and recommend changes to promote quality and competitiveness.
Act as the MHH/MH administrator on call as scheduled.
Assist in providing in-service training and orientation for agency staff.
Represent the agency at internal and external meetings.
Supervise all clinical services and meet department-specific metrics to promote quality care and control operational costs.
Assist the department leader and Operations team with achieving and maintaining compliance with quality improvement, risk management, and utilization review strategies.

Qualifications
Minimum:
Minimum of two (2) years of management/leadership experience (within the last 5 years) in a home health agency, primary care clinic or health facility, sufficient background knowledge and expertise in clinical decision making for the patient population assigned.
Current CA RN licensure and AHA BLS
Bachelor’s degree in a Health or Business related field
Preferred:
Master degree
