Job Summary and Responsibilities

As our Program Manager Hospice, you will oversee the overall operations and coordination of the Hospice Services program, ensuring the delivery of high-quality, compassionate, and patient-centered care. You will be responsible for program planning, staffing, resource allocation, budget management, and operational performance while ensuring compliance with organizational policies, regulatory requirements, and industry standards. Reporting to the Director of Home Health/Hospice, you will provide leadership and direction to clinical and clerical personnel while supporting the strategic goals and continued growth of the hospice program.

Every day, you will oversee program activities, coordinate staffing and scheduling, monitor patient census and acuity, and evaluate operational outcomes to ensure efficient use of resources. You will support staff development through training, coaching, and performance management while identifying opportunities to improve workflows, strengthen team collaboration, and enhance the patient and family experience. In addition, you will partner with interdisciplinary teams and leadership to address operational challenges, maintain regulatory compliance, and implement initiatives that support quality improvement and program effectiveness.

Success in this role will rely on your ability to combine strategic program leadership with operational excellence and a commitment to compassionate care. Through effective planning, data-driven decision-making, resource management, and collaborative leadership, you will help ensure the Hospice Services program delivers exceptional care that honors the comfort, dignity, and unique needs of patients and their families.

Job Requirements

Required

  • Bachelors Other - Business or Healthcare Administration or Equivalent; upon hire.
  • Minimum 5 years of demonstrated relationship management skills with internal and external partners.
  • Experience leading operational performance improvement initiatives and team engagement in both clinical and non-clinical settings.

Preferred

  • Two years of Home Health and/or Hospice experience in the last five years.
  • Project Management Professional (PMP).
