Responsibilities
The incumbent provides spiritual counseling and ministry to patients and their families, and functions as a member of the Interdisciplinary Team.
  • Meets discipline specific productivity standard.
  • Demonstrates competency in spiritual counseling with hospice patients/families during field observation and testing.
  • Completes and submits documentation following established guidelines as measured by a defined evaluation process.
  • Physician orders are written according to policy.
  • Documents follow-up of identified problems.
  • Reviews and documents clinical progress toward goals.
  • Demonstrates coordination of care by appropriate communication with other disciplines and documents that
    communication, as measured by a defined evaluation process.
  • Meets agency/regulatory requirements for inderdisciplinary care planning as measured by a defined evaluation process.
  • Documentation reflects changes in condition in support of Medicare Hospice benefit recertification decision.

Qualifications

Experience:

  • 1 year of Chaplaincy experience in the hospital or healthcare setting.

Licensure:

  • Certification or eligible for certification (with at least four units of Clinical Pastoral Education) as Chaplain by NACC or APC.
  • Certification as Chaplain by NACC (National Assn of Catholic Chaplains) or the APC (Assn of Professional Chaplains) preferred.
  • Current California Driver's License, possession of automobile insurance, and access to an operating motor vehicle.
  • Current BLS card.

Education:

  • Bachelor's Degree Advanced studies in Theology, Pastoral Care/Counseling, Spirituality preferred.
  • Masters degree in Theology or Divinity- preferred

Training:

  • 4 Units Clinical Pastoral Education

Special Skills:

  • Knowledge of: Hospice care; Principles of Death & Dying; Effective communication skills to work with interdisciplinary team; Beliefs and rituals of major religions; Mission, Philosophy, Values of Dignity Health and Ethical and Religious Directives for Catholic Health Care Services, Common Values for Community Sponsorship; Spiritual assessment; Ministry to sick in all age groups.
