Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

  • Abuse Protection and Reporting: Assesses and identifies and reports patient abuse and neglect i.e.: physical assault, rape or other molestation, domestic, abuse and neglect of elders and children and implements department policies and procedures. Measurment: Feedback, Medical Record Review, Observation.
  • Advance Directives: Consistently and accurately documents education regarding advance directives upon admission, coordinates efforts to obtain copy of Advance Directives to place on chart, and refers questions to Chaplain Services. Measurement: Documentation, Feedback/ Observation, Medical Record Review
  • Advocacy, Patient: Acts as the client's advocate, as circumstances require, by initiating action to improve health care or to change decisions or activities which are against the interests or wishes of the client, and by giving the client the opportunity to make informed decisions about health care before it is provided. Measurement: Observation, Feedback, Medical Record Review.
  • Color-Coded Wristband: Correctly identifies each color-coded wristband placed on patients on admission, assessment and reassessment because of specific risk factors/special needs and medical status. Measurement: Observation , Return Demonstration, Self-Learning Module Post Test, Discussion/Q&A, Quality Audit.
  • Cultural Diversity: Demonstrates ability to provide effective care, treatment, and services to an increasingly diverse population with complex needs. Measurement: Observation, Return Demonstration, Self-Learning Module Post Test, Discussion/Q&A, Quality Audit.
  • Documenting Care of the Client: Consistently and accurately documents care of the client as planned, implemented, and evaluated in the Medical Record using both hospital and department policies. Measurement:Complete Work Queues, Medical Record Review, Verbalization, Zero Leftover Events in clinical system.

Job Requirements

Required

  • One (1) year current experience in an acute or long term care setting required
  • Licensed Vocational Nurse: CA, upon hire
  • Basic Life Support - CPR, upon hire
  • Expanded IV Therapy Course, upon hire
