The Social Worker is responsible for the implementation of standards of care for Social Work Services in the Home Health and Hospice setting.



Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

  • Assesses the psychosocial status of clients related to the client’s illness and environment and communicates findings to the registered nurse.
  • Carries out social evaluations and plans intervention based on evaluation findings.
  • Maintains clinical records on all clients referred to social work.
  • Provides information and referral services to clients and families/caregivers regarding environmental needs and
  • Provides information to clients or families/caregivers and community agencies.
  • Markets and promotes the agency programs and events in the local community.

Job Requirements

Bachelor's degree in Social Work

Licensed as a Social Worker in Minnesota and North Dakota

One year experience in healthcare

Excellent verbal, written communication skills and organizational skills

Basic computer skills

Preferred

Master’s degree in Social Work - salary dependent on experience

Experience in Home Health and Hospice
