Care Coordination Assistant: Be the Hub of Patient Transitions!
Are you an organized, detail-oriented professional with a passion for helping people? Join our Care Coordination team as an Assistant, playing a vital role in seamless patient transitions from hospital to home, and beyond. Your efficiency and compassion will directly impact patient well-being and hospital operations.
In this essential role, you will:
- Coordinate Post-Acute Care:Manage timely referrals for patient placements, ensuring smooth discharges.
- Communicate & Collaborate:Be the key liaison between patients, families, agencies, and our clinical team.
- Ensure Continuity:Assist with follow-up appointments and necessary documentation to support patient recovery and prevent readmissions.
- Uphold Compliance:Work meticulously within hospital policies, HIPAA, and regulatory guidelines.
- Optimize Workflow:Support our RN Care Coordinators and Social Workers with critical administrative tasks.

Qualifications
Required
- Associate (two-year) Degree in Business/Health-Related field or High School Diploma or GED with a current license/certification as a healthcare professional (Medical Assistant/Certified Nursing Assistant/Emergency Medical Technician).
- Medical terminology certificate or ability to demonstrate knowledge of medical terminology is required.
- Intermediate to advanced computer skills required to include Microsoft Office and/or Google Workspace applications including Word, Docs, Excel, Sheets, typing, and data entry skills.
Preferred
- At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting.
- Bachelor’s degree or BSW.
- Experience supporting clinicians in demanding and fast paced patient care environments.
