Working closely with our Care Coordination and multidisciplinary teams, you'll be instrumental in:

  • Identifying and addressing social needs: From financial support to housing insecurity, you'll connect patients with vital community resources.
  • Supporting seamless discharge planning: Ensuring patients have the resources and support for a safe transition home.
  • Providing essential patient and family advocacy: Offering guidance and education during critical healthcare journeys.
  • Managing complex cases: Addressing sensitive issues like abuse, neglect, mental health, and end-of-life care.

Job Requirements

Required:

  • Bachelor’s degree from a CSWE-accredited social work program.
  • North Dakota Requirement: Licensed Baccalaureate Social Worker (SWORKER:ND).
