The posted compensation range of $30.38 - $37.95 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Responsibilities
The LVN Patient Navigator has a unique role in patient care. As a patient advocate, this individual will build working relationships, solve problems, direct patients to resources within the organization and externally in the community as well as support patients as they learn to self-navigate the healthcare system. The primary focus of the LVN Patient navigator is to support providers in the clinic and coordinate the plan of care with the healthcare team. The LVN Patient Navigator will assume a leadership role within the clinic for personnel to prepare and assist them with their responsibilities.
Qualifications
Required Education and Experience:
- At least 2 years experience working in a clinic setting with accountability for working with patients in nursing and/or case management/care coordination
- A minimum of 1 year experience in oncology
- Graduated from an accredited LVN program
- LVN:CA, BLS-CPR
Ability to listen attentively with empathy and capture essential information for continuity of care. Ability to move quickly from
one task to another independently. Ability to work collaboratively with all care professionals including physicians, nursing, and
external community agencies and post-acute care facilities. Ability to complete required data entry in a timely manner.
Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
