Responsibilities
  • Coordinate individualized wraparound services for clients and their families.
  • Assist clients with applications for Medicaid, SNAP, TANF, housing vouchers, and other public benefits.
  • Support clients in accessing transportation, child care, primary care, and behavioral health services.
  • Develop and maintain resource guides and referral systems for internal and community-based supports.
  • Collaborate with counselors, peer support staff, and external partners to ensure the delivery of integrated care.
  • Monitor progress toward service plan goals and track referrals and outcomes in documentation systems.
  • Advocate for clients’ needs in multidisciplinary team meetings and case conferences.
  • Conduct outreach and follow-up to enhance retention and reduce service gaps.
  • Maintain accurate, timely records and ensure compliance with all applicable policies and funding requirements.

Qualifications
  • Credential: Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
  • Experience: Minimum of 1 year of experience in case management, care coordination, or social services.
  • Knowledge: Familiarity with public benefit systems, housing programs, and community resources.
  • Skills: Strong organizational, communication, problem-solving, and advocacy skills.
  • Technological proficiency with electronic health records and basic office software.
  • Preferred: Bilingual in English/Spanish; experience working with underserved or recovery populations.

Work Environment and Physical Demands:

  • Office and community-based service settings.
    • Occasional local travel required for home visits, outreach, or accompanying clients.
    • Flexible schedule may include some evening hours to meet client needs.

The Case Manager at St. Joseph’s Recovery provides comprehensive case management and care coordination to support clients in recovery from substance use and co-occurring challenges. This position ensures access to essential services, including housing, healthcare, public benefits, and vocational resources, while promoting self-sufficiency and family stability through wraparound support.
