Responsibilities

The Care Coordination Assistant performs a wide range of complex and moderately-high-volume administrative tasks that support the patient’s hospital discharge process by the Care Coordination team.


Qualifications

Required:  

High school diploma or GED required. Excellent organization skills and aptitude for detail required. Strong written and verbal communication skills required. Strong interpersonal and customer service skills required. Good problem solving skills required. Must have ability to multitask, and ensure tasks are done within specified timeframe based on patient need. Skill in using Windows-based applications, and databases necessary. Ability to work independently under general supervision and make independent decisions. Must have the ability to work in a high-volume environment while maintaining high quality standards. Ability to independently manage customer service and project workflow. Ability to respond appropriately to moderately- to highly-complex issues in both standard and emergent situations.


Preferred: Associate’s degree. Two years experience in a healthcare setting, ideally in a discharge planning, utilization review, registration, billing or financial counseling capacity. Knowledge of durable medical equipment (DME) companies, criteria and authorization process. Familiarity with electronic medical record (EMR) and/or billing systems desirable.
