Job Summary and Responsibilities
Job Summary / Purpose
Enhances the patient experience by providing a seamless navigation process from diagnosis through the continuum of care to survivorship by offering individualized assistance to patients, families and caregivers to reduce barriers to care and facilitate timely access to care. Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate and consistently manage patient care by educating the patient and serving as a point of contact for patients and families. Works with patients and families to answer questions, provide emotional support and resolve any barriers, needs or issues related to the cycle of care. Responsible for serving as the patient's liaison and advocate in interactions with the entire treatment team.

Minimum Qualifications
Required Education and Experience | Education: Associates Degree, Nursing Experience: Five year’s nursing experience |
Required Licensure and Certifications | Registered Nurse License |
Required Minimum Knowledge, Skills, Abilities and Training | Organizational and Planning techniques. Effective Written and Verbal Communication. Time Management. Computer skills in database management and forms development. |
